- Duly completed Medical claim form
- Medical report or certificate, medical receipts or invoices, diagnostic report, prescription and other relevant documents
- A police report (in case of accident or death)
In case of death:
- ID/passport, family book, employment contract of the insured/deceased/legal personal representative
- An original or certified copy of the death certificate/cremation certificate
- An autopsy report
